(Erin):

Hi, I'm Erin, an oncology nurse practitioner, and today we're going to learn about the support that's available to you once you are prescribed ERLEADA®, also known as apalutamide. ERLEADA® is a prescription medicine used for the treatment of prostate cancer that has spread to other parts of the body and still responds to a medical or surgical treatment that lowers testosterone, or that has not spread to other parts of the body and no longer responds to a medical or surgical treatment that lowers testosterone. Please see additional important safety information later on in the video. Now you might be feeling overwhelmed. Don't worry. There are resources available for you as you start your ERLEADA® treatment.

Once you and your doctor decide that ERLEADA® is right for you, Johnson and Johnson offers a free personalized support program called ERLEADA withMe to help you with things such as: cost support options- available regardless of your insurance type, a free one-on-one dedicated care navigator support, additional resources, and community connections. Let's talk about cost support options. You might be wondering, what's ERLEADA® going to cost? Will insurance cover it, or how much will they cover? We don't want costs to get in the way of you getting the treatment you need, and that is why ERLEADA withMe can help you explore options to lower your out-of-pocket cost for ERLEADA®, no matter what type of insurance you have or even if you don't have insurance. They can help explain your medicine's, insurance coverage and potential out-of-pocket costs, and help find programs that might help you pay for ERLEADA®. If you have commercial or private health insurance, the ERLEADA withMe savings program may be able to help. You could pay as little as $0 per month for your ERLEADA® medicine. Note that the maximum program benefit per calendar year shall apply.

Savings may apply to your copay, your co-insurance, or deductible. This offer is subject to change or end without notice. You can participate without sharing your income information. See the program requirements at ERLEADA.JNJwithMeSavings.com. If you don't have commercial or private health insurance, they can provide information about other resources that may help you with your out-of-pocket medicine costs. These options include state-sponsored programs, Medicare Prescription Payment Plan, Medicare Part D, "Extra Help"–Low-Income Subsidy, Medicare Savings Program, Medicaid, and independent foundations.

Now let's review the support available to you through ERLEADA withMe beyond financial support. You can get connected to a dedicated care navigator who will partner with you one-on-one to give you the support you may need along the way. Let's say you want to know more about your disease. Maybe questions come up about ERLEADA® between visits to your healthcare provider. Maybe you're on the hunt for health and wellness resources for people living with prostate cancer. Or you're looking for an in-person support group. Maybe you need to be connected to transportation related services in your community. Or perhaps you're concerned about the cost of your treatment. Your care navigator has got your back on all of this. They are available from 8:00 AM to 8:00 PM Eastern every day, so they may be able to answer certain questions that arise when your doctor's office is closed. You or your care partner are both eligible for our Care Navigator as long as you're both age 18 years or older. Oh, and if you need Care Navigator support in other languages, just ask for translation service support when you sign up. Now that you know what the program can offer, let's review how you can enroll ERLEADA withMe so you can take full advantage of this comprehensive support program.

Here's how to get started. As soon as you and your doctor have decided that ERLEADA® is right for you, sign up for ERLEADA withMe online or by phone. Visit ERLEADA withMe.com and click sign up for support, or you can call 833-565-9631. Your support team is standing by. ERLEADA withMe is here to help you, so you can focus on being an everyday hero for your family and friends. The patient support and resources provided by ERLEADA withMe are not intended to provide medical advice, replace a treatment plan you receive from your doctor or nurse, or serve as a reason for you to start or stay on treatment. I hope this video helped you really understand how to get the support you may need to start and stay on treatment. Be sure to sign up for ERLEADA withMe as soon as you're prescribed your ERLEADA® so you can be well prepared for those conversations with your healthcare team and learn about the cost support options available to you.
